following is the office financial policy, carefully read the policy in its entirety.
If you have any questions, please call the business office for assistance. Vancouver OB-GYN accepts and will bill most major Insurance Carriers, this includes "open" DSHS and Medicare. Insured Patients Health Maintenance Organizations (HMO) - This type of plan requires a referral from your Primary Care Physician for all exams, excluding maternity care and annual exams. Without a written referral the charges become your financial responsibility. State and Federally funded plans (DSHS) - Vancouver OB-GYN accepts "open" medical coupons from DSHS patients. We will submit the claim for you. You must present a current medical coupon for services to be rendered. Payment will be required at time of service for those services not covered. Indemnity Plan (PPO) - PPO plans require that a deductible or percentage be paid by the policy holder. We will bill your insurance for the remainder. Maternity patients with a percentage plan will be asked to make payments throughout their pregnancy until the out-of-pocket amount and deductible has been met. Non Insured Patients Patients without insurance will be asked to pay at the time of service. If you are unable to, our Patient Financial Counselor will make payment arrangements with you. Please contact the Patient Accounts Office at (360) 892-9725 to make arrangements. Patient Responsibility Per office policy, all co-payments are due at the time of service. Insurance plan benefits and restrictions are the patient's responsibility. Our business office will need a copy of your current insurance card to submit your claims. Without this the charges may become your financial responsibility. In addition, we will need your updated information at each visit, including new insurance coverage, address, phone number, employer and emergency contact. Patient Billing Statements You will receive a patient statement showing the activity and balances due on your account. Please contact us if any problems arise. There will be a $25.00 fee for NSF checks (non-sufficient funds) and a $10.00 late fee for late payments. |
